Нексавар с активным веществом сорафениб – это противоопухолевый препарат (ингибитор протеинкиназы с кодом АТХ:L01XE05), который применяется при лечении некоторых злокачественных образований. Его используют для уменьшения раковых клеток. Часто используется для лечения рака печени и почек.
Нексавар (сорафениб) производится в виде двояковыпуклых круглых таблеток с пленочной красной оболочкой.
В одной таблетке присутствует:
- активное действующее вещество: сорафениба тозилат 274 мг (равно содержанию сорафениба 200 мг)
- также содержит: целлюлоза микрокристаллическая, натрия кроскармеллоза, гипромеллоза (5 cP), магния стеарат, гипромеллоза (15 cP), натрия лаурилсульфат.
Показания к применению Нексавар (сорафениб)
- Применяется при местно-распространенном раке, а также метастатическом дифференцированном раке щитовидной железы, резистентном к радиоактивному йоду.
- При печеночно-клеточном раке.
- При метастатическом почечно-клеточном раке.
Фармакокинетика препарата Нексавар (сорафениб)
При поступлении в организм вещества сорафениб его средняя относительная биодоступность составляет от 38 до 49 процентов. Время полувыведения действующего вещества составляет от 25 до 48 часов. Последующее использование препарата в течение недели после первого приема приводит к росту накопления в сравнении с приемом однократной дозы в 2,5-7 раз.
Равновесная концентрация препарата в плазме крови достигается в течение следующих 7 дней после приема препарата, отношение максимальной/минимальной концентрации равно не более 2.
Фармакокинетику активного вещества сорафениб в равновесном состоянии в дозировках 400 мг по 2 раза в сутки при приеме внутрь исследовали у пациентов с печеночно-клеточным раком, почечно-клеточным раком, а также раком щитовидной железы. Наиболее высокую экспозицию замечали у больных с раком щитовидной железы, хотя вариация экспозиции была большой при образованиях всех видов. У пациентов с раком щитовидной железы клиническое значение большей площади под кривой «концентрация — время» (AUC) не установлено.
Фармакодинамика препарата Нексавар (сорафениб)
Снижение пролиферации раковых клеток in vitro происходит по причине того, что действующее вещество Сорафениб является мультикиназным ингибитором.
Было установлено, что активное вещество Сорафениб подавляет многочисленные внутриклеточные киназы: BRAF, c-CRAF и мутантную BRAF, а также киназы, расположенные на поверхности клеткок, таких как: FLT- 3, KIT, RET, PDGFR-β, VEGFR-1, VEGFR-2 и VEGFR-3. Предполагается, что часть из этих киназ используются в сигнальных системах образований, в процессах ангиогенеза и апоптоза. Также вещество подавляет рост опухоли при дифференцированном раке щитовидной железы, печеночно-клеточном и почечно-клеточном раке.
Возможно применение с другими противоопухолевыми препаратами:
- Капецитабин
- Доксорубицин / иринотекан
- Неомицин
- Доцетаксел
- Омепразол
Упаковка, хранение, срок хранения Нексавар (сорафениб)
- Препарат производится в таблетках покрываемых пленочной оболочкой по 200 мг (28 таблеток в блистере). В пачку размещают по 1, 2 или 4 блистера с инструкцией.
- Хранить 3 года при температуре не выше 25°С в недоступном для детей месте.
