Мабтера (Ритуксимаб) — современное противоопухолевое средство (моноклональные антитела), выпускаемое в виде жидкости от светло-желтого цвета до бесцветного (от опалесцирующей до прозрачной), применяемое при лечении хронического лимфолейкоза, запущенной фолликулярной лимфомы и ревматоидного артрита (воспаление суставов).

Лекарственный препарат нацелен на взаимодействие с клетками CD20, которые являются важными для многих видов рака крови, а также распространенной формы лейкемии и лимфомы. Он уничтожает такие клетки самостоятельно, а также помогает бороться с ними иммунной системе.

Состав препарата Мабтера (Ритуксимаб)

В 1 мл содержится:

  • действующее вещество: ритуксимаб — 10 мг;
  • а также: полисорбат 80 — 0.70 мг, натрия хлорид — 9.00 мг, натрия цитрата дигидрат — 7.35 мг, хлористоводородная кислота или натрия гидроксид и вода для инъекций до 1 мл.

Активное вещество Ритуксимаб представляет из себя генно-инженерное химерное моноклональное антитело человека или мыши. Создается в виде мышиных вариабельных фрагментов легких и тяжелых цепей, а также гликозилированного иммуноглобулина IgGl с постоянным человеческим сегментом. Такие антитела получают из суспензии культуры клеток млекопитающего, а потом производят очистку с помощью ионообменной и аффинной хроматографии, которая включает специфическую вирусную инактивацию, а также процедуру удаления участков.

Показания к применению препарата Мабтера

Применяется при следующих заболеваниях:

Неходжкинская лимфома (НХЛ)

  • В комбинации с химиотерапией при фолликулярной лимфоме III-IV стадии, если ранее лечение не получалось.
  • Поддерживающее лечение после ответа на индукционную терапию у пациентов с фолликулярной лимфомой .
  • Монотерапия фолликулярной лимфомы III-IV стадии у пациентов с повторной химиотерапией или с рецидивом после химиотерапии, а также у больных с резистентностью к химиотерапии.
  • Неходжкинская лимфома (СD20-положительная диффузная В-крупноклеточная) совместно с химиотерапией по схеме CHOP (циклофосфамид, доксорубицин, винкристин, преднизолон).

Хронический лимфолейкоз (ХЛЛ)

  • Вместе с химиотерапией у больных не получавших терапию, а также у пациентов с рецидивирующим/рефрактерным ХЛЛ. Есть лишь ограниченные данные по эффективности и безопасности у больных, ранее уже получавших лечение моноклональными антителами, включая препарат Мабтера, или резистентных к предшествующему воздействию препаратом Мабтера совместно с химиотерапией организма.

Ревматоидный артрит

  • Совместно с метотрексатом при лечении у взрослых ревматоидного артрита с тяжелым течением и активной формой при неадекватном ответе, а также непереносимости иных базисных противоревматоидных лекарственных препаратов, изменяющих течение заболевания.
  • Также прием лекарственного препарата показал  уменьшение степени роста рентгенологически подтвержденного разрушения суставов, а также увеличение физической активности при использовании вместе с метотрексатом.

 

Микроскопический полиангиит (МПА) и гранулематоз с полиангиитом (ГПА)

  • Совместно с глюкокортикоидами у взрослых для индукции ремиссии при МПА и тяжелой активной формой ГПА (гранулематоз Вегенера).

Способ применения и дозирование препарата

Применение препарата Мабтера необходимо проводить под тщательным наблюдением грамотного медицинского работника с опытом в учреждении, в котором при необходимости, смогут незамедлительно провести реанимационные мероприятия.

Инструкция по обращению

Производится в апирогенных и стерильных одноразовых флаконах без консервантов. Флаконы содержат концентрат для приготовления раствора инфузий по 100 мг / 10 мл, а также 500 мг / 50 мл.
