Ибандроновая кислота, также известная как ибандронат натрия — это лекарственное  вещество, являющееся эффективным препаратом группы бисфосфонатов III поколения, применяемое для профилактики и лечения остеопороза, а также используемое для лечения гиперкальциемии (повышенного уровеня кальция в организме).

Активное вещество Ибандронат прописывают для профилактики и терапии остеопороза у женщин в постменопаузе. Действующее вещество утверждено и рекомендовано в качестве препарата для ежедневного лечения постклимактерического остеопороза. Базой для таких утверждений было тщательно проведённое исследование женщин с пост-менопаузным остеопорозом. Каждая участница также получала кальций и 400 МЕ витамина D. По результатам данного исследования, обе дозы значительно снизили риск (на 50-52 процентов) возникновения новых переломов позвонков по сравнению с действием препарата плацебо.

Состав

Одна таблетка содержит

  • активное вещество — натрия ибандроната моногидрата 168.75 мг. Объем эквивалентен кислоте ибандроновой 150.00 мг,
  • дополнительные вещества: целлюлоза микрокристаллическая, лактозы моногидрат, стеариновая кислота, повидон (К 25), кросповидон, диоксид кремния безводный коллоидный.
  • состав оболочки: опадрай 00А28646 (титана диоксид, гипромеллоза, тальк), макрогол 6000.

Показания к применению

Таблетки 150 мг:

  • Постменопаузный остеопороз для профилактики переломов.

Фармакодинамика

Ибандроновая кислота – высокоактивный азот-содержащий бисфосфонат, который способен специфически подавлять действие остеокластов, не влияя на их количество. Оказывает характерное избирательное воздействие на костную ткань, вследствие высокой схожести с минеральными компонентами кости, а именно с гидроксиапатитом, составляющим минеральную основу костной ткани. Активное вещество снижает резорбцию костной ткани, без прямого влияния на формирование костей, предотвращает костное разрушение, вызываемую блокадой функций половых желез. Не влияет на минерализацию костных тканей. У женщин в постменопаузе препарат снижает скорость деструкции костной ткани до уровня организма в репродуктивном возрасте, что приводит к нарастающему росту костной массы, в особенности позвоночника.

Фармакокинетика

При пероральном приеме препарат быстро всасывается в верхних отделах желудочно-кишечного тракта (ЖКТ). При приеме натощак через 0,5 – 2 часа достигается максимальная концентрация, а абсолютная биодоступность составляет 0,6 процента. Одновременный прием препарата с пищей или напитками (за исключением чистой воды) на 90 процентов снижает биодоступность. Прием напитков или пищи через полчаса после приема ибандроновой кислоты на 30 процентов снижает ее биодоступность. При приеме за один час до еды значимого уменьшения биодоступности не наблюдается.

Концентрация действующего вещества в крови повышается пропорционально принятой внутрь дозировки (до 100 мг) или введенного внутривенно (в дозировке до 6 мг) лекарственного препарата.

Распределение

После проникновения в кровоток активное вещество быстро связывается с костной тканью или выводится с мочой. Примерно 40–50% количества препарата, циркулирующего в крови, хорошо проникает в костную ткань и накапливается в ней. Кажущийся конечный объем распределения 90 л. Связь с белками плазмы крови 85%.

Метаболизм

Не существует пока данных о метаболизме ибандроновой кислоты.

Выведение

Около 40-50% циркулирующего в крови препарат выводится путем костной абсорбции, а остальная часть выводится в неизменном виде почками. Препарат,  невпитавшийся при пероральном приеме, выделяется в неизмененном виде с калом. Выведение ибандроновой кислоты имеет двухфазный характер. Терминальный период полувыведения составляет 10 – 60 часов. Общий клиренс составляет 130 мл/мин. Почечный клиренс равен 88 мл/мин, объем распределения равен 150 л. Различие между общим и почечным клиренсом отображат сколько вещества впиталось костной тканью. Концентрация препарата в крови снижается быстро и составляет 10% от максимальной через 8 ч после перорального приема.

Фармакокинетика у особых групп пациентов

Пациенты с нарушениями функции почек

Почечный клиренс при приеме ибандроновой кислоты линейно зависит от клиренса креатинина (КК). Для лиц с нарушением функции почек легкой или средней степени тяжести (КК ≥30 мл/мин) коррекции дозировки не требуется. У пациентов с тяжелым нарушением функции почек (КК <30 мл/мин), принимающих препарат в дозировке 10 мг перорально длительностью 21 день и более, концентрация активного вещества в крови в 2–3 раза больше, чем у лиц с нормальной функцией почек. При тяжелой степени нарушении функции почек, общий клиренс ибандроновой кислоты уменьшается до 44 мл/мин. Однако увеличение системной концентрации не ухудшает его переносимость. Требуется коррекция дозировки препарата у лиц с нарушением функции почек средней и тяжелой степени. Препарат выводится в процессе гемодиализа (37процентов в течение 4 ч).

Пациенты с нарушениями функции печени

Отсутствуют данные о фармакокинетике препарата у людей с нарушением функции печени. В клиренсе, когда препарат не метаболизируется печенью, а выводится через почки, печень существенной роли не играет. Поэтому для людей с нарушением функции печени коррекции дозировки не требуется.

Пожилой возраст

Изученные фармакокинетические параметры не зависят от возраста. Следует учесть возможное снижение функции почек у пожилых

Дети

Информация о применении ибандроновой кислоты у лиц моложе 18 лет отсутствуют.

Фармацевтические характеристики

Таблетки продолговатой формы. Покрываются оболочкой белого или почти белого цвета; с маркировкой на одной стороне: «150» .

Упаковка, срок годности, условия хранения

При производстве помещают по одной таблетке в контурную ячейковую упаковку из поливинилидихлоридной и поливинилхлоридной пленки. В картонной коробке размещается по одной контурной ячейковой упаковке вместе с инструкцией по применению.

Хранить при температуре не выше 30° С. Хранить в недоступном для детей месте!

Срок хранения 5 лет. Не использовать по истечении срока годности, указанного на упаковке
